How it works

1. My Lab — list your incubators, media, oil, consumables, equipment and techniques by name, once. Sixty rows pre-seeded; edit to match. The rest of the sheet picks from this list, so a CAPA says “Incubator 2 — K-Systems G210”, not “incubator”.
2. Event — the problem (pick from a list: fertilisation dropped, blastocyst rate fell, post-warming survival fell, incubator excursion, witnessing near-miss…), what was observed, severity, first noticed, recovered, KPI before / during / after, immediate containment.
3. Changes Log — everything you changed while troubleshooting, roughly dated: new lot, replaced, adjusted, serviced, stopped using, retrained… Mark each Helped / No effect / Unsure. The sheet computes days-before-recovery and stars changes made 0–7 days before recovery.
4. CAPA Form — writes itself: problem statement, the timeline of changes, root cause (the “Helped” change closest before recovery, for you to confirm), corrective action, factors investigated and excluded (everything that didn't help — recorded honestly as sequential elimination, which is exactly what an assessor wants to see), preventive action (pick from a library or write your own), 30-day effectiveness check, sign-offs. Print to PDF.

Why this order

Classic CAPA forms start with “state the root cause” — the one thing a busy embryologist doesn't know. Reasoning backwards from the changes turns a week of trial and error into a documented investigation. Nothing is invented; you confirm or overrule the suggested cause in your own words.
