What is in the file

• OPU sheet — one row per fresh cycle: date, your case code, age, AMH, then follicles, COCs, MII, 2PN, day-3 embryos, blastocysts, embryos frozen, embryos transferred and sacs.
• FET sheet — one row per frozen transfer: date, case code, age, AMH, embryos thawed, embryos survived, embryos transferred and sacs.
• Start Here — a short how-to and a plain-English definition of every column.

Why there are no calculations in it

A recording sheet should be quick to fill and impossible to break. Formulas get overwritten, copied wrongly between files and argued with. So the sheet stays raw, and the arithmetic happens on the website — the same way every time, for every month, against the benchmarks set for that lab. It also means you can keep the file open on a lab computer without worrying that a stray keystroke has changed a rate.

How labs use it

1. Keep one file per lab. Add each case as it happens — 20 seconds a row.
2. Upload it at My Embryologics → your lab, as often as you like. Months merge; re-uploading a month replaces it.
3. Read the dashboard: every rate by month and by year, green or red against your benchmarks, with a score and a printable report for your clinic owner.

Privacy, by design

The sheets have a Case Code column, not a Patient ID. Use your own internal reference. When you upload, only monthly totals are stored — never a case row, never a case code.

Benchmarks

Default targets are set by Medical Interventions with reference to the Vienna consensus and ESHRE performance indicators. They are starting points, applied per lab on the dashboard — and any workbook that carries its own benchmark table still overrides them on upload.
